Non inclusion in the obligation of Tfillin (e.g. someone who has no arms) or the obligation of Tefilla (e.g. in the case of someone who is Osek BeTzarchei Tzibbur or an Onen) is not the same as a "blockage" that prevents one who is obligated from fulfilling his Chovat Gavra. An ones (mitigating circumstances) will daven a Tefillas Tashlumin. The melacha of Kevuras HaMes is included in the Isurim of Y"T Sheini. This confrontations must be resolved. The Gra"m Solo believed that Kevura is not included in Isurei Y"T Sheini. The Gra"ch taught that Pikuach Nefesh is not a confrontation; there is no Shabbos in this situation!
