Tosfos (Bishikadum) cites Rabbaynu Ephraim who distinguishes between a Drop of Chaliv vs a Drop of Milk Falling on Kosher Meat. There are 2 stages of contamination. 1st, piece of meat A receiving the contamination. 2nd, subsequently A is mixed with other kosher pieces of meat. If milk falls into meat A, then the entire volume of A is prohibited. Not just the milk needs nullification in the 2nd mixture, but the entire volume of piece A. However, if chaliv (forbidden fats) contaminates A, only the chaliv absorbed in A needs nullification. Rabbaynu Tam argues and says the entire volume of piece A is prohibited and not just the chaliv needs nullification in the 2nd mixture. This is the famous disagreement regarding Chanan bShar Issurim. But both agree that Chanan occurs with milk & meat.
